Note: Your name is currently registered as a Sole-owner/ Co-owner in SLA’s property ownership.
Supporting Documents to be attached with the application
1. Your NRIC (front only) showing your full name, NRIC number and photo
2. Your current Property Tax Statement which contains your full name (as in your NRIC) and the property address as Documentary Proof of Ownership
3. Type 1 Authorization Form completed with your signature

2. Are you a Staff form a Registered Proprietor of a Corporation / an Organisation?
Note: Name of the Corporation / Organisation which you are working with is currently registered as a proprietor in SLA’s property ownership record – This includes the religious organisation / non-profit organisation

1. Your NRIC (front only) showing your full name, NRIC number and photo
2. Corporate Owner’s Authorisation Form authorizing you to buy plans
3. Current Property Tax Statement which contains your Corporate Owner’s full name (as in Corporate Owner’s Authorisation Form) and the Property address as Documentary Proof of Ownership
3. ACRA Biz File of Corporate Owner
4. Type 2 Authorisation Form completed with your signature

3. Are you the chariman of Management Corporation Strata Title (MCST)?
